If you are unable to apply online, you can download an application form (SA1)
You can complete the Word version of the form and return it by email to firstname.lastname@example.org or you can print a version, complete by hand and return to your child’s current school. You can also contact the School Admissions and Appeals Team and we will send a copy of the form to you.
However you choose to apply, you must submit your application by the closing date of 15 January 2014 – if your form is late it may affect your chances of being allocated a place at one of your preference schools.

At the time of publication, the government had not confirmed that Chetwynde School’s application to become a ‘free school’ from September 2014 had been approved. Parents who want to apply for places at Chetwynde School should complete two application forms. One of these should be the school’s own application form, which is available below and also on the Chetwynde School website.
A further online application or form SA1 listing three preferences, not including Chetwynde School, should also be completed and returned to the School Admissions and Appeals Team by the deadline date.
Applicants who can be offered a place at Chetwynde School will receive that offer and will also be offered a place at an alternative school. This will ensure that, in the absence of final confirmation that Chetwynde’s bid for free school status has been approved by offer day, 16 April 2014, all parents can be certain that their child has a school place for September 2014.
